WHAT IF I HAVE TO CANCEL MY APPOINTMENT?

Our clinic has a 24-hour cancellation policy. A 24HR cancellation notice is required on all appointments. Failure to cancel prior to 24HR before your appointment is a $50 charge, if you are more than 15 minutes late you may be re-appointed. Any outstanding account balances must be paid in full upon each office visit. Please be sure that you are aware of what your health insurance provider will cover for our services and what you will be responsible for.

WHICH HEALTH INSURANCE PROVIDERS DO YOU ACCEPT?

We accept Cigna, Aetna, BCBS, United Health Care, UMR, Humana Tricare, Anthem. We also provide cash payment options to all patients. Please call our office for more information if you have questions.

WHO WILL BE RESPONSIBLE FOR FMLA PAPERWORK?

FMLA paperwork is completed on a case-by-case basis. While we do typically have a three-day turnaround policy, Dr. Adeogun must have seen you consistently for between 6 and 12 months prior to completing any FMLA paperwork. A charge may apply for completing FMLA forms.

AM I THE RIGHT PATIENT FOR YOUR CLINIC?

Dr. Adeogun has experience working with patients ages 8 and up.

HOW DO I REQUEST A MEDICATION REFILL?

If you need a medication refill, please allow yourself at least a 24-hour window. In some cases, Dr. Adeogun may not be able to respond to your request for a refill right away. As a result, it’s important to plan. If you do not get an answer when reaching out to us via telephone, please be sure to leave a detailed voicemail with your name, date of birth, and the name of the medication. We will contact you as as soon as possible.  If it is a medical emergency, please call 911.

DO YOU OFFER PSYCHOTHERAPY?

We do offer supportive therapy. We can assist with referral for patients needing psychotherapy.

WHAT SHOULD I BRING WITH ME TO MY FIRST APPOINTMENT?

Once you have fully completed all the consent forms in the patient portal, be sure to bring with you proper identification, proof of insurance and a list of all the medications you are currently taking.

WHAT IF I HAVE A QUESTION ABOUT MY TREATMENT PLAN OR MEDICATION?

If you have a question about your treatment, please do not hesitate to reach out to us during normal business hours. If you attempt to reach us via telephone and do not receive a response, please be sure to leave a detailed message. We stay firmly committed to responding to all communication within 24 hours.
